Intake Form

An intake form is a digital questionnaire the client completes before their first spray tan appointment. Standard fields: Fitzpatrick skin type, current medications (some retinoids increase DHA absorption), pregnancy status, recent waxing or laser, recent sunburn, allergies (some clients react to fragrance in lower-tier solutions), preferred solution depth, prior spray tan experience, and explicit acknowledgment of the studio's cancellation policy. The intake also includes a liability waiver acknowledging that DHA spray tan provides no UV protection. Bronzly's intake form builder lets each studio customize the questions and routes flagged answers (pregnancy, eczema, recent waxing) to the artist's pre-appointment view so they can adjust technique. Intake forms are sent via SMS link 24 hours before the first appointment; subsequent appointments only require a re-acknowledgment of the cancellation policy.
